Lack of effective leadership.
The night before the launch of the space shuttle "Challenger", the chief engineers of the rocket boosters recommended not to launch. Their own management overruled them. The result was a disaster.
This programme investigates the various pressures on NASA management to launch the shuttle, the obsessions of the engineers and the internal debates which led up to the decision to launch the shuttle on 28th January 1986.
This disaster cost the lives of the crew. It lost prestige for the USA internationally and seriously slowed their space programme. It also had an effect upon schools across the world because crewmember Christa McAullife was a teacher who had planned to broadcast a special lesson from space.
