Lack of co-ordination in response to a crisis. An insightful programme which investigates the disaster to the oil rig Piper Alpha and draws out lessons which can be applied elsewhere. This tile is used in health and safety training by organisations across the world.
The explosion and fire on the oil platform Piper Alpha was not an unpredictable event. If the adjacent oil rigs had been given the freedom to cut production, most of the damage would have been prevented and lives could have been saved.
