A Crash in the Dark investigates, when two trains collided in the Severn Tunnel due to faulty equipment and human error, a complete communication breakdown meant that the rescue teams walked for miles in the wrong direction, unable to find the crash site.
The British Rail safety regime was severely criticised because the control centre informed their railway superiors of the incident but did not alert the emergency services. Another failure occurred when a train was assembled to take the emergency crews into the tunnel to carry out a rescue, but the train driver refused to move the train because he did not have authority to do this.
